In particular, you need to upload a PDF document of your honors thesis (this is where you can carve out what you contributed to other papers). Requirements for the document are listed online (https://undergrad.cs.umd.edu/honors/requirements). 

After we receive this form, we will ask your (UMD CS) faculty advisor to approve your honors thesis, and eventually at the end of the semester, we will check your final GPA and coursework requirements have been satisfied.
